Prior to enrollment we will need all necessary forms completed. These forms provide emergency information and your child's history that is needed for a safe, secure, educational, and fun environment. Other information needed may include:

  • recent immunizations
  • a health statement or other doctor's notes
  • permission forms
  • additional food program forms

Please speak with the front office if there is any information on your forms that needs to be updated such as a change of contact information or others authorized to pick up your child. All forms must be updated annually.
